Job description

Gloucestershire Health and Care NHS Foundation Trust

This is an exciting opportunity to continue to support the development of the rehabilitation pathway for individuals with mental health difficulties residing in Gloucestershire.

The Community Rehabilitation Team was set up in 2024 as a pilot project and has achieved success in supporting individuals living with psychosis to lead more fulfilling lives. The STAR team look to support individuals both in community and inpatient settings as they oversee a variety of functions, including receiving and responding to requests for funding for non-commissioned services, Personal Health Budgets & s.117 Aftercare funding requests. They have achieved success in reducing the number of out of area placements in recent years.

Main duties of the job

As Rehabilitation Pathway Manager you will provide management oversight to the Community Rehabilitation Team and STAR Team. The successful candidate will look to work collaboratively with other key stakeholders to further develop and embed the rehabilitation pathway within Gloucestershire for adults experiencing mental health illnesses.

Person Specification
Experience
  • Significant management experience of working with people with a mental health condition in a variety of settings including the community.
  • Experience of working with people with a mental health disability and mental health problems or behaviours which challenge.
  • Experience of working with service users with complex care needs requiring higher levels of expertise.
  • Experience of audit, research and the application of evidence-based practice.
  • The ability to be self-motivated and work independently in managing their own workload and commitments.
  • The ability to make informed decisions that are evidence-based and devise care plans that may have a serious impact upon service users/carers and the wider community.
  • To have experience of forensic mental health issues and experience of working with mentally disordered offenders and restricted patients.
  • Experience of partnership working.
  • Knowledge of and ability to apply relevant Mental Health Act & Mental Capacity Act legislation.
  • Knowledge of principles of mental health rehabilitation - community and inpatient.
  • Experience of budget management and managing large budgets (in excess of £5,000,000).
  • Ability to participate in service development processes at a strategic level.
  • Relevant professional Nursing Qualification/Social Work/Occupational Therapy OR equivalent extensive experience working within the public sector in a Leadership position.
  • Accredited post-registration training/qualification (equivalent to Masters level) relevant to people with mental health difficulties.
  • Masters Degree.
  • Management Qualification.
Specialist Knowledge
  • Understanding of case management within mental health services.
  • Awareness of best and evidence-based practice in relation to mental health disability.
  • Ability to contribute to the development of a better model of care for people with MH problems in Gloucestershire.
  • Ability to case manage a large number of clients and ensure their needs and care packages are regularly reviewed to maximise independence and empowerment.
  • Working knowledge of Continuing Health Care eligibility criteria.
  • Significant experience and knowledge of Personal Health Budgets.
  • Understanding of commissioning/contract management.
  • Computer literacy and ability to contribute to the development and maintenance of a database designed to support invoicing, regular reviews and service/policy analysis.
  • Ability to negotiate consensus in relation to assessment and care planning/care packages.
  • Working knowledge of Responsible Commissioner guidance.
  • Can demonstrate ability to develop and deliver creative approaches to service provision.
  • Up to date clinical knowledge including risk assessment, dual diagnosis and complex health and social care needs.
  • Comprehensive knowledge and understanding on the Mental Health Act, MCA and legal aspects of Mental Health Care and s.117 Aftercare.
  • Awareness and understanding of contemporary development in mental health care including the Recovery Model.
