An exciting and challenging opportunity has arisen for a full-time and fixed term contract (6 months) band 6 Physiotherapist or Occupational Therapist within the Integrated Independence Team.


We are looking for an innovative, enthusiastic and experienced Physiotherapist or Occupational Therapist to be part of the A& E and community rapid response service within the Integrated Independence Team. This will be offered as permanent and full-time for an existing band 6 therapist or experienced band 5 therapist.


The Integrated Independence Team is an integrated health and social care service provided jointly by Homerton University Hospital NHS Foundation Trust and London Borough of Hackney. The team places the user at the heart of the care planning process, and aims to promote independence, deliver outcome focused care, and provide a seamless journey through the care system.


The team provides rapid response, intermediate care therapy and reablement services and consists of Occupational Therapists, Physiotherapists, Psychotherapists, Nurses, Speech and Language Therapists, Consultant Geriatricians, Social Workers, Independence Coordinators, Independence and Specialist Independence Assistants and Administration staff. The team will use integrated support planning to address health and social care needs across the pathway in a responsive, dynamic and flexible way.


Working Hours: 37.5 (part time, flexible working or compressed hours will be considered)


Main duties of the job:


We are looking for a Band 6 Physiotherapist or Occupational Therapist to provide a rapid response, senior Physiotherapy/Occupational Therapy assessment and treatment of complex patients within A& E, short stay unit and patients referred from the community in order to prevent admission to hospital and promote early discharge.


In doing this you will work as part of a strong multidisciplinary team, within an interdisciplinary working model, to facilitate hospital discharge and prevent acute admission.


The ideal candidate will have strong experience as a Physiotherapist or Occupational Therapist in an Acute Therapy or Rapid Response setting. You should have excellent communication skills, a proactive approach, prioritisation and problem solving skills.


You will be encouraged to develop your clinical, teaching and management skills through opportunities within the team and in the wider organisation. You will have support from a Clinical Specialist Lead and experienced staff across the team. Peer supervision and in service training are carried out on a regular basis and you will have an annual appraisal. There is excellent support for personal development and opportunities to develop the service in line with best practice. You will have the opportunity to support and supervise junior staff within the service, and to support service development and projects with the team.

Essential:

  • Degree / diploma in OT recognised by RCOT or CSP
  • HCPC registration

  • Able to manage a clinical caseload and to prioritise as required.

  • Demonstrates effective communication skills with a variety of agencies to optimise patient / client management.

Experience:


Essential:

  • Post graduate experience in general physical field of OT or physiotherapy
  • Experience of working in acute hospital setting
  • Experience in contributing to service development projects.
  • Experience of working in a community setting.

Knowledge/Understanding:


Essential:

  • Demonstrate application of: -Assessment and treatment approaches in physical rehabilitation -Occupational Therapy/Physiotherapy process including discharge planning Application of Models of practice
  • Awareness of current professional and NHS issues relevant to the client group.
  • Risk management / clinical governance principles
  • Outcome measures and other quality issues, including clinical effectiveness
