Job Title: Social Worker – Hospital Discharge Team
Location: Liverpool
Rate: Up to £43.78 per hour
An exciting job role has recently become available in Liverpool. They are looking for a new member for their Hospital Discharge Team.
What will your responsibilities be?
You will manage a complex caseload with supervision, ensuring adherence to policy, procedures, and legislation. Your responsibilities will include preparing reports and representing clients at Court hearings and Court of Protection applications. You will make complex decisions and manage risk using professional judgment and evidence-based tools. Effective communication in high-risk situations and representing the department in multi-disciplinary forums are key aspects of this role. You will conduct direct work with service users, carers, and families following safeguarding policies. Additionally, you will assess and navigate care for individuals with eligible social care needs, collaborating with health agencies. Planning and delivering services to meet assessed needs within financial constraints, monitoring support plans, and participating in safeguarding enquiries, capacity assessments, and best interests assessments, ensuring timely completion, are also essential responsibilities.

To be successful in this role, you must have Social Work England registration, be eligible to work in the UK, hold a qualification in Social Work at degree level, and have post-qualified experience.
