We are currently looking for a passionate and experienced Adult Social Worker to join our team. Key Responsibilities:

• Experienced social worker required to work in the Integrated Discharge Team based in Whiston Hospital. This will entail assessing patients and other tasks associated with facilitating hospital discharge, to support patients with the support they require to return home or to be discharged to another setting.
• The role is very fast paced and high pressured and experience in this area would be preferred.
• Conduct assessments to determine the care and support needs of vulnerable adults, including older people, individuals with disabilities, and people with mental health needs.
• Develop, implement, and review care plans that are person-centered, promoting independence and well-being.
• Provide support for adults to enable them to live as independently as possible, ensuring that they have access to the right services and interventions.
• Safeguard vulnerable adults, working closely with families, carers, and other professionals to address risks and promote safety.
• Undertake reviews of care plans to ensure they continue to meet the needs of individuals, adjusting care packages as necessary.
• Work within a multi-disciplinary team, collaborating with health services, housing providers, and other relevant agencies to provide a holistic approach to care.
• Keep accurate and up-to-date records, ensuring that documentation complies with legal and council requirements.
• Provide advice and support to families and carers, empowering them to better support the adults in their care.
