Position: Qualified Social Worker – Adults Team
Location: Medway Council
Salary: £38ph
Contract Type: Full-Time and on-going contract

About the Role:

We are looking for an experienced and dedicated Social Worker to join our Medway Adults team. In this vital role, you will work with adults who require support to live independently, providing essential social work services to promote their well-being and enhance their quality of life.

Key Responsibilities:

  1. Conduct comprehensive assessments of adults’ social care needs and develop tailored care plans.
  2. Provide support to individuals and families, ensuring access to necessary services and resources.
  3. Work closely with healthcare providers, local authorities, and community organizations to coordinate and deliver services.
  4. Safeguard vulnerable adults by identifying risks and taking appropriate action to ensure their safety and well-being.
  5. Offer advice, guidance, and advocacy to service users, helping them to make informed decisions about their care.
  6. Maintain accurate records in line with statutory requirements and local policies.
  7. Contribute to multi-disciplinary team meetings and work collaboratively with other professionals.
  8. Ensure that all interventions are person-centered and promote independence, choice, and control for service users.
