What you will do:

1. Conduct assessments and reviews to ensure individuals' needs are effectively met, focusing on promoting independence and well-being.
2. Identify and facilitate access to reablement opportunities and relevant services.
3. Apply a strength-based approach to help individuals assess their own needs and access community resources.
4. Monitor ongoing service provision and ensure effective coordination to maintain focus on outcomes and well-being.
5. Provide professional advice and support to individuals, carers, and partner organisations.
6. Perform duties under the Mental Capacity Act and safeguarding responsibilities as a Safeguarding Officer.
7. Develop skills by maintaining a social work caseload with appropriate supervision.
8. Contribute to health and social care assessments, including continuing healthcare assessments, once trained.
