Qualified Social Worker - Adults needed in Wirral UK, earning £33.97 hourly.

This is an ongoing locum opportunity for a full-time role with a fantastic team spirit and a supportive work culture. You will be reporting to the Team Manager or Advanced Practitioner and will play a key role in delivering essential health and social care services for adults and carers in the Wirral community. This position is vital to implementing the Personalisation and Inclusion Pathways, ensuring all around compliance with the Care Act 2014. What you will do:

  1. Work as part of an integrated team to plan, monitor, and evaluate collaboration with the Multi-Disciplinary Team (MDT).
  2. Coordinate signposting and referrals in collaboration with colleagues and external services.
  3. Work within social work ethical frameworks, providing support to adults, carers, and families to promote self-determination and autonomy.
  4. Support duty functions within the Review Team, participating in case triage and team meetings.
  5. Assist with Multi Agency Safeguarding Hub (MASH) and related safeguarding processes.
  6. Undertake Best Interest Assessments for Deprivation of Liberty Safeguards (DoLS).
  7. Conduct needs-led assessments, complex case work, and initiate multi-disciplinary assessments.
  8. Support adults, carers, and families, involving them in planning to promote resilience and prevent hospital admissions.
  9. Adhere to professional standards and ethical frameworks in all duties.
  10. Communicate complex information and care plans effectively while ensuring confidentiality.
  11. Prepare funding applications and collaborate with health colleagues and stakeholders.
  12. Engage with community forums to promote positive social work practices.
  13. Undertake enquiries for adults at risk, ensuring personalised responses.
  14. Organise and chair multi-disciplinary meetings, providing guidance as necessary.
  15. Challenge societal stigma and discrimination impacting individuals with disabilities.
  16. Provide advice and information on mental health and social care matters.
  17. Contribute to training development for staff and partner agencies.
  18. Mentor and support junior staff in their professional development.
