Description:

Part-time/Full Time PRIVATE Dentist required 2-3 days per week in a busy, FULLY PRIVATE well-established practice near Saltash, Cornwall.

1. Full NHS Performer Number.
2. Local Area Team confirmation.
3. GDC certificate.
4. UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years.
5. Indemnity certificate.

7. Enhanced DBS (within the last 12-36 months).
8. Clinical references covering places of employment over past 2 years (minimum 2 referees).
9. Degree certificate.
