Remuneration: Rate @ 50%

Description: Part-time PRIVATE Dentist required for 2-3 days a week including Thursday late evening 10.00-18.00 and Saturday 09.00-14.00 in a busy well-established, INDEPENDENTLY OWNED practice in Chippenham, Wiltshire.

1. 50% split on Private & Labs
2. BDA Good Practice member
3. Modern, fully computerised practice, SOE
4. iTero Scanner, CEPH scanner, Digital x-rays, Rotary endo, OPG
5. Excellent support team including hygienists on-site
6. In-house Orthodontist and endodontist
7. Dentist will be managing a fully private list of Fee Per Item and Denplan.
8. Opportunity to increase days as demand dictates.
9. On-site parking available
10. 30 minutes from Bath, 35 minutes from Bristol and 30 minutes from Swindon
11. Available now, start date negotiable

Compliance requirements:

1. Full NHS Performer Number
2. Local Area Team confirmation
3. GDC certificate
4. UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
5. Indemnity certificate
6. Passport
7. Enhanced DBS (within the last 12-36 months)
8. Clinical references covering places of employment over past 2 years (minimum 2 referees)
9. Degree certificate
10. CPR/AED/Medical Emergencies certificate dated within the last 12 months
11. IRMER
12. PIN number
