Job description

Surrey Lodge Group Practice is a 2-partner practice with an interesting & diverse population of around 10000 patients. We are developing our motivated and supportive team and are looking to recruit a Practice Nurse. We would welcome a Nurse at any stage of their career. We have a very experienced clinical team with 10 GPs, Pharmacist, ANPs, Nurses and HCAs. We also have a very supportive and friendly administrative team.

We are looking for a candidate who is enthusiastic, dedicated and can demonstrate the ability to give a friendly, caring and professional nursing service to our patients. In return we aim to offer an excellent work/life balance. This role will include cytology, asthma and COPD management, diabetes management, childhood/adult immunisations, requesting Pathology Tests, for example Urine Culture, Swabs, and Chronic Disease Management and evaluation of care. They will work collaboratively with the General Practice Team to meet the needs of our patients.

Job responsibilities

Provide Primary Care Nursing Services as follows:

  • Follow up of patient test results and communication with patients where appropriate.
  • Phlebotomy Services as required.
  • Home Visits where required.

Provide timely Clinical and Culturally appropriate Health Services through:

  • Ongoing Health Education and advice, including counselling to improve health and prevent disease.
  • Use of recall and reminder systems and, as appropriate, referral to National Screening Programmes.
  • Education to assist patients to reduce or change risky and harmful lifestyle behaviour.
  • Delivery of immunisation programmes for both Adults, Children and Babies.
  • Travel Services, including Vaccinations and advice.
  • Working with Public Health providers in the prevention and control of communicable diseases for individuals and families and reporting to relevant Public Health Providers.
  • Ongoing care and support for people with Chronic and Terminal Conditions.
  • Health promotion to the practices registered population, linking to Public Health Programmes at a Local, Regional and National level, and where appropriate, using such programmes to target specific populations.
  • The Supervision and Assessment of Student Nurses and Junior Colleagues (when fully trained to do so).
  • Assist with the provision of information for Practice Reporting requirements as requested.
  • Ensure that the Emergency trolleys are checked regularly and appropriately equipped with up to date drugs.
  • Ensure that all information entered on to the EMIS is accurate, proportionate, appropriate and in accordance with Practice Policy.
  • Attend Practice Meetings and play an active part, including the dissemination of information to the wider Nursing Team.
  • Undertake responsibility for own professional Nursing Development and attend for regular Clinical Supervision.
  • Understand and agree that the Confidentiality of both Patients and other Team Members is maintained.
  • Comply with Health and Safety and Infection Control Policy and Guidelines.

Person Specification

Qualifications

  • Registered Nurse
  • Post graduate diploma or degree (other)
  • Post graduate diploma or degree (Primary Care)

Experience

  • Experience of chronic disease management
  • Experience of working in a primary care environment
  • Experience of working as a practice nurse or community nurse

Clinical Knowledge & Skills

  • ECGs
  • Venepuncture
  • New Patient and other medicals
  • Chaperoning support
  • Requesting pathology tests and processing the results, advising patients accordingly
  • Understand importance of evidence based practice
  • Broad knowledge of clinical governance
  • Ability to record accurate clinical notes
  • Ability to work within own scope of practice and understanding when to refer to GPs/other clinicians
  • Awareness of issues within the wider health arena
  • Knowledge of health promotion strategies
  • Immunisations (routine, childhood and travel)
  • Women's health (cervical cytology and contraception etc)
