A reputable, patient-centered medical practice in Leyton is looking for a skilled and compassionate Practice Nurse to join their dedicated healthcare team. The ideal candidate will contribute significantly to the provision of exceptional patient care within a diverse community.
Key Responsibilities:
Deliver comprehensive nursing care, including management of chronic conditions, wound care, immunizations, and health screenings.
Conduct patient assessments, develop individualized care plans, and support the delivery of patient-centered services.
Work closely with GPs, healthcare assistants, and the multidisciplinary team to ensure a seamless continuum of care.
Provide patient education and health promotion advice, empowering patients to actively manage their health.
Maintain accurate, up-to-date records, adhering to relevant clinical guidelines and protocols.
Qualifications and Experience:
Registered Nurse (RGN) with a current NMC pin.
Experience in a primary care or community healthcare setting preferred.
Proficiency in managing chronic conditions, including diabetes, asthma, and COPD.
Strong communication skills with the ability to work collaboratively in a team environment.
Competence in using electronic patient records and clinical systems.
