Job Summary:

A reputable, patient-centered medical practice in Leyton is looking for a skilled and compassionate Practice Nurse to join their dedicated healthcare team. The ideal candidate will contribute significantly to the provision of exceptional patient care within a diverse community.

Key Responsibilities:

1. Deliver comprehensive nursing care, including management of chronic conditions, wound care, immunizations, and health screenings.
2. Conduct patient assessments, develop individualized care plans, and support the delivery of patient-centered services.
3. Work closely with GPs, healthcare assistants, and the multidisciplinary team to ensure a seamless continuum of care.
4. Provide patient education and health promotion advice, empowering patients to actively manage their health.
5. Maintain accurate, up-to-date records, adhering to relevant clinical guidelines and protocols.

Qualifications and Experience:

1. Registered Nurse (RGN) with a current NMC pin.
2. Experience in a primary care or community healthcare setting preferred.
3. Proficiency in managing chronic conditions, including diabetes, asthma, and COPD.
4. Strong communication skills with the ability to work collaboratively in a team environment.
5. Competence in using electronic patient records and clinical systems.
