A well-established and forward-thinking GP practice in Harrow is seeking a dedicated and compassionate Practice Nurse to join their friendly and supportive team. The ideal candidate will have a strong commitment to providing high-quality patient care and a passion for working within a community-focused healthcare environment.

Key Responsibilities:

1. Provide a comprehensive range of nursing services, including chronic disease management, immunizations, wound care, and health screening.
2. Conduct patient assessments, including taking medical histories and supporting the diagnosis and management of conditions.
3. Offer advice and support on lifestyle and health issues, such as smoking cessation, weight management, and family planning.
4. Collaborate with GPs, healthcare assistants, and administrative staff to ensure seamless patient care.
5. Maintain accurate and detailed patient records in line with practice protocols and NHS guidelines.
6. Participate in the development and implementation of practice policies and procedures.

Requirements:

1. Registered Nurse with current NMC registration.
2. Previous experience in a primary care or community setting is highly desirable.
3. Strong communication and interpersonal skills, with the ability to build rapport with patients and colleagues.
4. Ability to work independently and as part of a multidisciplinary team.
5. A proactive approach to professional development and a commitment to staying up-to-date with current clinical practices.
