Practice Nurse (Diabetes Interest)

Job summary

We are recruiting for an experienced Practice Nurse to join our Nursing Team and co-ordinate the programme of care for our diabetic patients.

Our Practice Nurses are supported by our Lead Nurse and deliver nursing services within the boundaries of their scope of practice to the practice population. The Practice Nurses are responsible for several clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure. They work collaboratively with our teams to meet the needs of patients and provide excellent and safe patient care. Our nursing team work across 3 of our patient-facing sites within Waterlooville.

We are looking for a motivated team player who is highly organised, enthusiastic and patient-focused with a can-do attitude and friendly personality. Candidates must have evidence of Diabetes training (Diploma, PitStop or other accredited course).

We can consider candidates seeking 30 - 37.5 hours per week between Monday- Friday, with a working pattern to be agreed. Extended hours on a rotational basis will form part of the working pattern (after successful completion of the probationary period).

Main duties of the job

To focus on the co-ordination, and provision of diabetes clinics and referrals for patients. To support colleagues with complex diabetes queries and help to develop the diabetes strategy.

To work as part of our nursing team to provide a diverse and comprehensive range of nursing services and care to our patient population.

To support the following clinics/specialities and provide hands on, autonomous patient care:

• Wound Care
• Leg Ulcer Clinics
• Long Term Condition Management (Hypertension, CHD, CKD, Diabetes, COPD/Asthma)
• Cervical Cytology
• Childhood and Adult Immunisations
• Travel Health

A commitment to maintain a high professional standard of nursing care and to undertake ongoing training and routine skills updates in order to deliver services.

Job responsibilities

• Assist and perform tasks related to patient care as directed by Lead Nurse and GPs following agreed clinical protocols.
• Deliver evidence-based care and structured education to people with diabetes, their families and other health colleagues in a primary care setting.
• Participate in local Diabetes Steering Groups, particularly looking at clinical standards, guidelines, pathways and improving outcome measures.
• Lead the internal diabetes meetings, updating colleagues on new NICE and local guidance and sharing prescribing knowledge.
• Provide holistic assessment of wounds and implement wound care including complex wound management.
• Support NAs/HCAs carrying out wound care by maintaining clear care plans.
• Identify, manage, and support patients at risk of developing long-term conditions.
• Maintain chronic disease registers and assist with QOF searches and reporting.
• Maintain accurate clinical records in conjunction with Practice policies and current legislation.
• Ensure read codes are used effectively.
• Ensure safe storage, rotation and disposal of vaccines and medication.
• Support the clinical team with all safeguarding matters.
• Provide appropriate self-care advice for patients presenting with minor illness.
• Recognise and manage anaphylaxis, according to the current UK Resuscitation Council guidelines.
• Perform Cardio-pulmonary Resuscitation and the correct use of Automated Defibrillator.
• Manage emergency drug kit and perform weekly checks as appropriate.
• Undertake the collection of pathological & microbiology specimens.
• Travel Health Consultations.
• Womens Health including Cervical Screening.
• Provide contraception advice and sexual health guidance.
• Offer advice about childhood and adult immunisations.
• Provide chronic disease clinics, delivering patient care as necessary.
• Deliver opportunistic health promotion.
• Chaperone and assist patients where appropriate.
• Provide information and advice on prescribed or over-the-counter medication.
• Request pathology tests.
• Process pathology results as required and act on them appropriately.
• Prioritise health problems and intervene appropriately.
• Actively participate in all aspects of infection prevention and control.
• Maintain and clean equipment and clinical rooms.
• Attend and participate in practice meetings as required.
• Actively take part in mandatory and internal/external training as requested.

Person Specification

Experience Essential

• Experience as a Practice Nurse in Primary Care.
• Experience of managing Diabetes as a primary condition.
• Experience of chronic disease management.

Knowledge & Skills Essential

• Clinical skills and knowledge: Wound care including compression bandaging, Urinalysis, ECGs, Venepuncture, Hypertension, Cardiovascular disease primary prevention, Coronary Heart Disease, Diabetes, Routine immunisations.
• Excellent organisational and time management skills.
• Ability to work as a team member and autonomously.
• Excellent communication skills.
• Full UK Driving licence with own transport.
• Sound IT skills.

Qualifications Essential

• NMC Registered Nurse.
• Level 6 qualification or above in Diabetes Care / Management.
