Main duties of the job

Job Description:

We are looking for an experienced and motivated Practice Nurse to join our experienced team to deliver a range of nursing services to our patients. The successful candidate will work closely with our GP's, healthcare professionals and administrative staff within the practice to support and deliver a high standard of patient care.

Key Responsibilities

  • Conduct comprehensive assessments and develop care plans for patients with chronic diseases.
  • Monitor and review patients' conditions, adjusting care plans as necessary.
  • Provide education and support to patients and their families on managing their conditions.
  • Administer vaccinations and immunisations.
  • Perform cervical screening and family planning services.
  • Collaborate with GPs and other healthcare professionals to ensure coordinated care.
  • Maintain accurate and up-to-date patient records.
Requirements
  • Registered Nurse with valid NMC registration.

About us

We are a friendly, forward-thinking GP practice based in Chipping Norton. We are fortunate to benefit from a strong supportive and well-established workforce set on the edge of the Cotswold. The building is a modern, purpose-built facility providing primary health care to local residents and those in the surrounding rural areas as well as local nursing and residential homes and a community hospital.

We are looking for an experienced Practice Nurse to complement our existing nursing team.

Job responsibilities

The post holder will provide general practice nursing duties including chronic disease management, immunisations (adhering to current national guidelines), performing cervical smears (adhering to current cervical cytology register requirements) and run general nurse-led clinics.

Main Duties and Responsibilities:

  • Ensure that patients receive high quality clinical care, delivered in a timely manner.
  • Ensure complete and accurate documentation of each and every patient contact.
  • Work as an independent practitioner and as part of multidisciplinary and multi-agency teams in order to ensure patients' needs are met.
  • Provide clinical advice to Reception staff where required.
  • Offer support to junior staff and learners.
  • Ensure that as a nursing team we are prioritising people, practising effectively and safely and promoting professionalism as per nursing code of conduct.
  • Participate in defining own developmental plan to maintain and develop own clinical skills and ensure own professional development.
  • Undertake and complete all statutory and mandatory training.
  • Participate in an appraisal and regular performance reviews.
  • Support the development of the HCAs.
