Job description

An excellent opportunity has arisen for an experienced and enthusiastic Practice Nurse to work at Grovelands Medical Centre in Reading.

You must be willing to work as part of a team and provide excellent clinical care and support to our patients in a varied role. You must possess a calm, confident approach; have good communication skills and the ability to perform the relevant duties well.

Experience in Chronic Disease Management, Cervical Cytology, Family Planning, Immunisations & Treatment Room Duties are essential.

Main duties of the job

To be responsible for the delivery of practice nursing services, and work as part of the practice's multidisciplinary team. You will be expected to provide care within your scope of practice to the entitled patient population. The practice nurse will be responsible for a number of clinical areas such as health promotion, chronic disease management, health prevention, as well as supporting the management team in the reviewing of clinical policy and procedure.

Job responsibilities

  1. Clinical
    1. To manage patients with chronic disease, running specific clinic sessions and maintaining robust call and recall systems. Treatment duties.
    2. Participate in national screening work such as cervical cytology and deliver the national immunisation and vaccination schedule.
    3. Provide travel health advice and administer appropriate vaccinations.
    4. Treat patients with minor injuries.
    5. Provide directly and assist GPs with contraceptive services.
    6. Act as chaperone when required.
    7. Undertake health promotion to help educate patients.
    8. Select, carry out and interpret results of specific tests.
    9. Undertake anticoagulation clinics to include interpretation of blood tests and dosing, advised by the Lab.
    10. Provide general treatment room duties (Ulcer/Doppler etc) including assessment, planning, implementing and reviewing care.
    11. Provide Chronic Disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required.
    12. Implement vaccination programmes for adults and children.
    13. Recognise, assess and refer patients presenting with mental health needs.
  2. Administration
    1. To maintain accurate patient records and enter onto the computer using agreed Read Codes.
    2. Ensure accurate completion of all necessary documentation associated with patient health care.
    3. Support the clinical team with safeguarding matters, in accordance with local and national policies.
    4. Request/process pathology services as required.
    5. To collect data and participate in regular Practice audit and research projects.
    6. To contribute to the preparation of any practice development plans.
  3. General
    1. To maintain own annual registration with the NMC and adhere to the Code of Professional Conduct.
    2. To maintain a safe and therapeutic environment for all staff, patients, carers, and co-workers from other agencies. Ensure all statutory regulations (e.g., Health & Safety, COSHH, etc.) are implemented and adhered to.
    3. To always promote a professional approach by attire and attitude to patients and staff both in and out of the Practice.
    4. To maintain own continuous professional development and ensure PDP is kept up to date in line with the requirements of the Practice.
    5. Attend and participate in staff and Practice meetings and in-house clinical training sessions.
    6. Any other reasonable duties that may be required from time-to-time.

Person Specification

Qualifications

  • Registered Nurse
  • Postgraduate diploma or degree (Primary Care)

Experience

  • Experience of working in a primary care environment
  • Experience of working as a practice nurse or community nurse
  • Experience of chronic disease management especially Diabetes.
