Parkgate Surgery are looking to recruit a Practice Nurse to join our busy GP practice.

We are offering a part-time role of approximately 20 hours per week, to be worked over 2-3 days.

Main duties of the job

Candidates should be experienced in all areas of Chronic Disease Management, Cervical Cytology and Vaccination and Immunisation. A specialist interest in Diabetes and COPD would be desirable.

You will work with the full support of the practice team together with benefitting from the substantial networking opportunities across our other practices in the area. This is a great opportunity to join a well established team.

The ideal applicant will have experience in the Quality and Outcomes framework and other targets required of modern day general practice, confident with IT and management of processes in a rapidly changing environment.

You will have excellent communication skills, a compassionate and friendly nature who can operate in a busy environment with a range of people and professions, as you will be working with our clinical and administration teams.

About us

Parkgate Surgery is a 5 GP partnership led Practice. We are based in Ormskirk, a market town and home to Edge Hill University in West Lancashire.

We are a large surgery across 2 sites with nearly 10,000 patients.

We have a high prevalence of frail and elderly patients, of which we employ our own Frailty Nurse to complete home visit anticipatory care management plans for our patients.

We are a digitally forward-thinking practice with implementation of modern access to General Practice through our total triage system and cloud-based telephony system.

We have recognised the live challenges of balancing and meeting patient demand but also ensuring the wellbeing of our staff by holding frequent full Practice Wellbeing Meetings and keeping in regular contact with our PPG.

We house Medical Students from the local university and recently qualified for being a training practice and enjoy having GP registrars at our surgery.

We are part of a successful PCN Group and utilise, support and mentor ARRS Roles.

We received Outstanding on our last CQC visit and frequently get full points on QOF.

We have our own Care Co-ordinator who is the first point of call for all our Palliative Care patients, ensuring they receive all the care they need.

We conduct a weekly in-house MDT meeting with local agencies which is very successful.

Job responsibilities

  1. Develop, implement and embed health promotion and well-being programmes
  2. Identify, manage and support patients at risk of developing long-term conditions, preventing adverse effects on patients’ health
  3. Implement and evaluate individual treatment plans for chronic disease patients
  4. Request pathology services as necessary
  5. Process pathology results as required
  6. Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
  7. Maintain accurate clinical records in conjunction with extant legislation
  8. Ensure read codes are used effectively
  9. Maintain chronic disease register
  10. Chaperone patients where necessary
  11. Assist GPs with minor surgery when required
  12. Prioritise health issues and intervene appropriately
  13. Support the team in dealing with clinical emergencies
  14. Recognise, assess and refer patients presenting with mental health needs
  15. Implement vaccination programmes for adults and children
  16. Support patients in the use of their prescribed medicines or over-the-counter medicines (within own scope of practice)
  17. Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients etc.)
  18. Delegate clinical responsibilities appropriately (ensuring safe practice and that the task is within the scope of practice of the individual)
  19. Understand practice and local policies for substance abuse and addictive behaviour, referring patients appropriately
  20. Deliver opportunistic health promotion where appropriate
  21. Provide routine nursing care to patients as required in accordance with clinical-based evidence, NICE and the NSF
  22. Support the clinical team with all safeguarding matters, in accordance with local and national policies

Person Specification

Qualifications

  • Registered Nurse
  • Postgraduate diploma or degree (Primary Care)

Experience

  • Experience of working in a primary care environment
  • Experience of working as a practice nurse or community nurse
  • Experience of chronic disease management

  • Excellent communication skills (written and oral)
