Qualifications

  • Registered with the NMC and substantial experience as a Practice Nurse.
  • Knowledge of managing long-term health conditions and familiarity with PGDs.

What we’re looking for:

We are looking for a candidate with experience managing patients with a range of long-term health conditions, who is adaptable to change and confident working autonomously. You will be keen to work within non-traditional care models and committed to your continuous development.

You should be eager to undertake relevant training, support your nursing colleagues, and share your passion for helping patients lead healthier lives. Your expertise in health prevention, screening, and early management of long-term conditions will be key.

Key Responsibilities:

As a Practice Nurse, your responsibilities will include:

  • Venepuncture
  • Blood pressure monitoring
  • ECGs
  • Height/Weight/BMI measurements
  • Health promotion
  • Childhood and adult immunisations
  • Wound care
  • Contraception management
  • Cervical screening
  • Spirometry
  • Management of long-term conditions, including Diabetes, COPD, and Asthma
  • Experience with long-term condition management and cervical screening is essential.

Essential Requirements:

  • Registered with the NMC
  • Degree in Nursing
  • Substantial experience as a Practice Nurse
  • Minimum of one Long-Term Condition diploma
  • Knowledge of patients with long-term conditions
  • Familiarity with Patient Group Directions (PGDs) and associated policies
  • Ongoing CPD (Continuing Professional Development)
  • Ability to produce accurate, contemporaneous, and complete records in line with legislation, policies, and procedures.
