What you will do:

1. Conduct community visits to assess patients recently discharged from hospitals ensuring a smooth transition back to home life.
2. Develop individualised physiotherapy plans tailored to the specific needs of each patient.
3. Collaborate effectively with other healthcare professionals to provide holistic care.
4. Motivate and educate patients on exercises and recovery best practices to promote their independence.
5. Regularly monitor and report patient progress making adjustments to treatment as necessary.

Requirements:

1. Applicants must be registered with the Health and Care Professions Council (HCPC) and have a minimum of two years of experience in a relevant field.
2. A minimum of 2 years of experience in community Physiotherapist is preferred.
3. Possessing your own transport is essential to perform community visits efficiently. We are seeking someone who is flexible, adaptable, and eager to face new challenges head-on.
