Job Title: Band 6 Physiotherapist

Location: Taunton

Start Date: ASAP

End Date: Ongoing

Responsibilities:

• The Physiotherapist requires community experience within adult reablement.
• The Physiotherapist must be a car driver and owner. Although the service works in geographical clusters, the individual should be willing to travel cross county if required to meet patient needs.
• The D2A service provides short term reablement following hospital admission within people's own homes.
• The Physiotherapist will work alongside D2A trained provider agencies to deliver assessment and reablement.
• The Physiotherapist will set patient-led SMART goals to progress independence and recommend longer-term care and support needs.
• Qualified Physiotherapist

These posts are for a minimum of 6 months.
