Community Physiotherapist Role
Location: Taunton

Requirements:
- The Physiotherapist requires community experience within adult reablement.
- The Physiotherapist must be a car driver and owner. Although the service works in geographical clusters, the individual should be willing to travel cross county if required to meet patient need.
- The D2A service provides short term reablement following hospital admission within peoples own homes.
- The Physiotherapist will work alongside D2A trained provider agencies to deliver assessment and reablement.
- The Physiotherapist will set patient led SMART goals to progress independence and recommend longer term care and support needs.
- Qualified Physiotherapist.
These posts are for a minimum of 6 months.
