Are you a qualified Physiotherapist who wants to make a difference? Do you want to be involved in the continued development of an innovative integrated service?

As an autonomous member of a well-supported therapy team within Nottingham CityCare Partnerships Integrated Care Homes Service, your aim will be to optimise clients' independence and prevent hospital admissions.

As an NHS Health Professional, you will provide integrated holistic care to a wide range of clients, by in-reaching into Nottingham City Care homes to deliver NHS services. The service's overarching aim is to ensure that care home residents and staff have an improved experience through high-quality care and access to familiar health professionals with the skills to meet their needs.

Your caseload will comprise adults living in care homes who have either complex physical health needs or are living with dementia. In addition to your proven physical therapy skills, you will have the opportunity to develop your knowledge and skills working with professionals from the specialist services, who in-reach into care homes giving you invaluable CPD to support your continued learning and development.

Main Duties of the Job

This Physiotherapy role is part of the therapy element of the Integrated Care Home Service. This element comprises both physical health and dementia specialist therapists and Assistant Practitioners, working as an integrated therapy team, supported by a therapy lead.

All referrals to the therapy element of the service are triaged by one of the therapy team and assigned to the most appropriate therapist for the patients' needs.

The Physiotherapist is required to possess a high level of clinical knowledge and up-to-date experience in this area of work. They are responsible for the holistic assessment of patients and the prescribing of a programme of therapy for individual patient requirements. They will hold a clinical caseload.

There are wider therapy meetings, led by the clinical lead, with therapists from other CityCare services who in-reach into the care homes. These meetings enable a sharing of knowledge and the development of new and innovative ways of working.

The Physiotherapist will be involved in contributing to the evaluation and audits of service provision. They will be required to attend meetings and partake in working groups (as appropriate) in which knowledge regarding therapy interventions to residents in care homes is required, and to represent the wider service as required.

Job Purpose

  • To assess, plan and develop treatment programmes for Nottingham City patients within care homes who have a wide variety of complex physical needs, to maximise the physical and social environments to help people engage in activities of daily living and contribute to multi-disciplinary treatment planning.
  • Determine clinical diagnosis and physiotherapy treatment indicated and to maintain records as an autonomous practitioner.
  • To be responsible for continuing the ongoing development of high standards of clinical practice and expertise within the service contributing to implementation of specific changes to practice and service protocols and making a professional contribution to the work of a multi-disciplinary team.
  • To advise and support care home staff in maintaining a residents' wellbeing and engagement.
  • To manage/care coordinate a caseload of patients, and will provide advice, education and teaching sessions to providers of residential/nursing care.

Special Requirements

Essential
  • Friendly, approachable, stays calm under pressure, diplomatic, leadership skills, recognises own limitations.
  • Able to manoeuvre patients using handling aids, able to carry out manual handling assessments of patients in care homes, able to kneel, bend and stoop, and work in cramped environments.
  • Car driver and possession of a current full UK driving licence.
Desirable
  • Working knowledge of Systm1.
QualificationsEssential
  • BSc (Hons) Physiotherapy or equivalent.
  • HPC Registration.
Desirable
  • Clinical Educators / Practice Teaching Course.
  • Relevant post-graduate experience.
ExperienceEssential
  • Relevant post-graduate experience working with adults and older people.
  • Experience of multi-disciplinary and/or multi-agency working.
  • Competence in individual caseload management.
Desirable
  • Community-based practice experience.
Knowledge and SkillsEssential
  • Evidence of achievement of appropriate competencies for clinical grade.
  • Awareness of current Local and National Policy Developments and remains update with professional practice and current best evidence and research.
  • Physiotherapy diagnosis/assessment and treatment skills.
  • Awareness, understanding and commitment to the pursuit of equality and diversity in terms of service delivery and employment practice.
  • Competent use of information technology appropriate to role.
  • Experience of supervision and appraisal of less experienced qualified staff and student/support workers.
  • Contribution to clinical education of less experienced staff/students and support workers.
  • Excellent communication skills demonstrating an ability to communicate to a variety of audiences and levels, e.g. 1:1 and in groups, teaching.
  • Knowledge and experience of a wide range of approaches to the management of complex conditions demonstrated by clinical reasoning.
  • Experience of change and change management including dealing with conflict.
  • Remain updated with professional practice and current best evidence and new research, and evidence of CPD.
  • Flexible, adaptable, innovative and self-reliant with a positive attitude.
  • Good organisational skills, ability to prioritise and delegate.

  • Evidence of audit/organisational projects.
  • ECDL or equivalent.
