We are looking for an experienced Physical Therapist. This is a part-time, 30hrs/wk position working Monday-Friday between the hours of 8am-6pm, but may require flexibility with office hours. York Hospital has Physical Therapy office locations in York, Wells, South Berwick and Kittery, Maine.

Responsibilities:

1. Providing physical therapy services to patients in an outpatient setting in accordance with a written Plan of Care approved by a physician.
2. The Physical Therapist makes an initial evaluation and plans with patient and physician for ongoing physical therapy services.
3. Initiates, reviews, and implements the PT Care Plan for each patient based on the assessment and prescribed Plan of Care.
4. Revises Plan of Care when indicated.

Minimum Requirements:

1. Minimum of (1) year of Physical Therapy experience.
2. Outstanding customer service.
3. Computer skills.
4. State of Maine PT License.
5. Basic Life Support (CPR) Certification.
