Outline of role:

As my Personal Assistant, your role is to provide me with the help that I need. This means that it is my responsibility to tell you exactly what I need, when I need your help, and how I want you to help me. It is your responsibility to ask what I need, to listen, and respond to any reasonable instructions that I give you.

The types of tasks that I might need help with are listed below. I will tell you when and how I need your help. This list is not exhaustive, and I may need help with things that are not listed below. Provided my request is reasonable, I will expect you to follow my instruction. If you are unclear about anything, you should ask.

I am a 35-year-old male who has, from birth, been diagnosed with severe quad spastic Cerebral Palsy. I exhibit autistic behaviours but have no formal diagnosis. Therefore, I need support on a 24/7 basis with all aspects of daily living. I live at home with my dad who is now my sole carer since October 2023 when my mum passed away, who had been my primary carer. I like to watch all types of DVDs and explore the personal lives of the actors and actresses. I also like to go out to places of interest and have lunch out often meeting friends and family.

Tasks:
Personal tasks - each day:
  1. Help with dressing
  2. Help with showering/washing
  3. Help with shaving
  4. Help with oral hygiene
  5. Help in and out of bed
  6. Help with using the toilet
  7. Help with getting from bed to chair/wheelchair and vice versa
  8. Help with all aspects of using slings and hoists for transfers and movements in the house
Household tasks - on a regular basis, as requested by me:
  1. Cooking and preparing meals and drinks
  2. Keeping the kitchen clean and tidy
  3. Laundry - washing, drying, and ironing
  4. Changing bedding
General tasks - as and when I ask:
  1. Going to the local shops/assisting me to go to the local shops to get supplies
  2. Assisting me to go to therapy appointments and local places of interest using wheelchair and WAV
  3. Engage with me in my interests and socialise with me
Social activities - as and when I ask:
  1. Assisting me in participating in various social activities, as per my instructions, including going shopping, social activities, and meeting friends.

This is not a list of everything that I may ask you to do. I may ask you to do similar sorts of tasks which are not listed above. Provided that my request is reasonable, I expect you to follow my instruction.
