The perioperative care coordinator is an exciting, new, evolving role. This is a result of new NHS England best practice guidelines for pre-screening and optimisation of perioperative patients. Getting it Right First Time (GIRFT) recommends that NHS organisations should establish Perioperative Care Co-ordination teams, to introduce early preoperative screening, risk stratification and provide an opportunity to optimise patients' health prior to surgery.

The post holder will support the Trust in adhering to national guidance and develop learning for Cheshire & Merseyside ICB. This post will include using digital tools, such as C2Ai and Graphnet, to identify patients requiring optimisation ahead of planned surgical intervention as well as those who may be suitable for triage to a high-volume, low complexity site. Training will be provided around personalised care, use of healthcare specific IT systems and on brief intervention conversations.

Main duties of the job
  1. Contacting all patients newly listed for surgical procedure to ask or ensure they have completed a pre-screening questionnaire, and where required support them to complete the form.
  2. Collate information at the point of referral from primary care and in the pre-screening questionnaire.
  3. Use the above information and digital intelligence to support the initial triage of those likely to require optimisation.
  4. Contact patients to inform them of outcome of pre-screening triage, and where appropriate and under the direction of a qualified health professional, use approved materials to provide them with universal advice about pre-operative preparation, including diet, exercise/activity and healthy living.
  5. Under the guidance of a registered health professional, arrange appropriate follow up appointments for higher risk patients.
  6. Use this knowledge to refer patients to their most appropriate local services. Examples will include activity classes, weight loss and psychological support.
  7. Refer patients to health promotion services, for example smoking cessation, alcohol reduction and diabetes control.
  8. Where relevant, encouraging patients to find out if their GP has a social prescribing link worker and/or health coach, and how they can refer themselves.
  9. Quality assure surgical prioritisation coding in line with Royal College guidelines and where necessary, escalate and stream cohorts of patients to the relevant clinicians and services.

Qualifications
  • NVQ Level 4 or equivalent
  • Educated to GCSE standard with passes at grade C or above for English and Maths or similar through vocational training
  • Evidence of commitment to continued learning or improvement through attendances at courses
Knowledge & Experience
  • IT literate with good keyboard skills
  • Previous experience of working within a health or social care setting or alongside these sectors (including unpaid work)
  • Experience of using initiative and willingness to challenge appropriately
  • Experience of using Information technology (MS Teams, Outlook, Word, PowerPoint, Excel) and electronic notes systems
  • Ability to analyse and manage conflicting demands and prioritise workload
  • Awareness of the need to maintain confidentiality
