An exciting opportunity has arisen for a Clinical Pharmacist to undertake a pioneering role within primary care.
Working as part of a team which currently comprises of three Clinical Pharmacists and a Pharmacy Technician. The PCN is open to applications from newly qualified Pharmacists.
You will work closely with all PCN and practice staff.
To work as part of a multi-disciplinary team in a patient-facing role.
Provide clinical leadership on medicines management and quality improvement as well as managing some aspects of the quality and outcomes framework and enhanced services.
Primary Duties and Areas of Responsibility includes but not limited to: Long-term condition Clinics, Structured Medication Review, medicines support, medicine information to practice staff and patients, unplanned hospital admissions reviews, management of medicines at discharge from hospital, repeat prescribing, risk stratification, service development, information management, medicines quality improvement, medicines safety, implementation of local and national guidelines and formulary recommendations.

We are interested to hear from candidates looking for full or part-time roles to fill our vacancies. All applicants will be required to hold current GPhC registration as Pharmacist. Previous experience of working in a GP practice is desirable but not essential. Applicants will need to be computer literate, confident in working independently, able to prioritise workload and want to work in a multi-disciplinary environment.
