Children's Nurse Case Manager - Perm Job Offer!

Place of work: Devon-Newton Abbot- Office based

We offer: £50-55K (Full time) per annum - depending on experience, 23 days annual leave + bank holidays, pension, salary sacrifice schemes, benefits programme, free parking, and more!

Working hours: Full time, Part time, Condensed hours

Responsibilities

What will you be responsible for:

Coordinating comprehensive care for children with complex health conditions, ensuring they receive the right services.

Specifically the activities will be:

• Assessment
• Care plan development
• Coordination with providers
• Patient advocacy
• Excellent communication

Experience

What requirements are we looking for?

Experience in complex care management is a plus!
