We are looking for an enthusiastic, dynamic, adaptable and forward-thinking experienced RN to join OPAL service. The Older Person's Assessment and Liaison service provides a multidisciplinary, patient-centred comprehensive assessment to frail and elderly patients presenting with an acute need and supporting them through virtual services.
The post holder will work closely with teams from the acute and community settings. They will be a strong role model who provides leadership and direction to the service along with the multidisciplinary team.
The post holder must have excellent communication skills and experience of managing complex and challenging situations and demonstrate clinical judgement and critical decision-making skills based on evidence-based practice. An ability to prioritize workload and be able to use initiative while working as part of a team.
Part of the role is to identify suitable patients for the virtual ward by working closely with the multidisciplinary team. Virtual wards allow patients, where possible, to receive safe care within their own home, ensuring the provision of excellent care with compassion for the patients and relatives. There will be close links with consultant geriatricians and the wider multidisciplinary team involved in the care of these patients.
