Applocum are currently recruiting for ANP/ACP's for a Liverpool based Home Visiting Service.
System: Adastra (Training can be provided)
Home Visiting Sessions: *Driver is Provided*
As part of this service, you will be completing home visits across the Locality. 30 minutes is allocated for each appointment. This is for adult patients only and is classified as an Acute Home Visiting Service; however, experience in palliative care would be a great addition as generally, patients are homebound and over 60 years of age.
Free parking is available at the base site where the shifts start.
Shift Patterns:
- 7 Day a week service covering times when GP Practices are unable to accommodate Home Visits.
- Monday – Friday shifts on a regular basis – 7pm – 11pm shifts
- Saturdays and Sundays – 9am – 1pm, 10am – 3pm, 3pm – 6pm etc.
