An established industry player is seeking a compassionate and skilled Band 4 Nurse to join their dynamic Older Peoples Medicine team. This role is crucial in supporting patient care for those over 65, particularly those with complex medical needs. The successful candidate will work collaboratively within a multi-disciplinary team, ensuring a high standard of individualized care and assisting in the management of bed flow. With a commitment to excellence and patient-centered care, this position offers an exciting opportunity to make a meaningful impact in a supportive and innovative environment. Join a workforce that values respect, integrity, and dedication to patient care.
Are you a clinical Band 4 Nurse who is looking for a new challenge? Older Peoples medicine (OPM) is a specialty that has evolved in recent years to meet the needs of the growing population of people over the age of 80 years old.
It holds the first Older People's Emergency Department (OPED), which provides emergency care to patients in an environment dedicated to meeting the needs of the client group.
Older Peoples Ambulatory Care (OPAC) works alongside OPED and our short stay ward Loddon to provide same day emergency care and short stay medicine.
OPM's 4 remaining wards each have a unique specialist focus within the specialism. Brundall and Dunston Wards are acute wards that care for patients requiring a prolonged stay due to complex medical conditions and discharge planning. Elsing Ward is an acute ward caring for patients with complex cognitive impairment and delirium.
Earsham is an ortho-medical ward providing care for patients who are over 65 years who have had a fractured neck of femur (NOF), with a background of multiple comorbidities.
OPM prides itself on its multi-disciplinary team working to provide every patient with a high standard of individualised care, with a goal of safely discharging patients home or to a placement suitable to meet the needs of the patient when they are medically fit to leave the acute trust. OPM is praised for its high standard of care and support to patients and their families at the end of their life.
We are looking for someone to work with our front door services and our acute OPM wards to support the management of bed flow through the department and to support the teams with initial nursing assessments when a decision to admit a patient has been made.
There couldn't be a better time to join our team as we begin to change the way we triage our patients using the Rockwood frailty score and moving from Older Peoples Medicine to Frailty.
