What you will do:

  • Work with adults aged 18 years and upwards with physical disabilities, learning disabilities, acquired brain injuries, and chronic health conditions.
  • Complete thorough strengths-based reviews and engage in mental capacity act assessments and Continuing Health Care checklists and decision support tools.
  • Be part of a dynamic team, contributing to powerful strengths-based assessments utilizing your Occupational Therapy skills to develop and implement wellbeing-focused support plans.
  • Provide advice, information, assistive equipment, and recommendations for home adaptations.
  • Apply evidence-based practice to inform your interventions, carrying out risk assessments and necessary assessments under adults' legislation.
  • Contribute to Continuing Health Care assessments, providing expert knowledge with a flair for multidisciplinary collaboration.

To excel in this role, you must be registered with the HCPC and possess a full driving license as you will need to travel within the area.
