What you will do:

  1. Deliver safe and effective rehabilitative patient care, coordinating effectively with colleagues in both community and acute sectors to optimise resource use.

  3. Work flexibly across localities and at weekends to meet service requirements, maintaining a strong presence within the team.

  5. Engage with patients in setting their rehabilitation goals, ensuring informed consent is gained while working within the legal framework for those lacking capacity.

  7. Evaluate patient progress, adapting treatment programmes as needed, and develop comprehensive discharge plans ensuring seamless care continuity.
