As a Nurse Case Manager, you will:

1. Manage a caseload of clients, acting as the main point of contact for clinical updates.
2. Develop, implement, and review care plans and risk assessments.
3. Conduct regular reviews of client care plans and documentation.
4. Ensure safe management and administration of medication.
5. Safeguard children and vulnerable adults, escalating concerns as necessary.
6. Monitor, support, and coach care staff, providing on-call support as needed.
7. Promote best practices and provide training to enhance care quality.
8. Liaise with external professionals and multi-disciplinary teams.
