Do you have a passion for transforming the lives of people who have experienced severe injury or illness? Here at the National Rehabilitation Centre (NRC) you will join an innovative, forward-thinking team with the collective objective of providing first-class rehabilitation to our patients.
The role of the NRC case manager is integral to the patient journey and interdisciplinary team. This role drives the coordination of admission, management of the patient journey and discharge from the NRC whilst maintaining a key liaison role for families and carers. The post holder will be experienced in senior healthcare leadership, coordination of patient pathways, aligning professional and patient expectations and driving timely clinical decision-making. The post holders will promote the clinically excellent model of rehabilitation at the NRC ensuring strong and visible clinical leadership throughout the patient journey.

Your contribution will be vital but not only that, you will be supported to grow and learn through our many developmental programmes at the NRC. The NRC is in the advanced stages, and the building will be ready in the summer of 2025. This role will be temporarily based at our current unit Linden Lodge at City Hospital until we move to the 70 bed NRC facility in summer 2025.
