Job description

Elm Tree Surgery is seeking a newly qualified GP (within 2 years of qualification and without a substantive post) who is enthusiastic and forward-thinking to join their team. The position offered is a fixed-term salaried contract, ideally beginning immediately.

The postholder's role will primarily involve patient-facing clinical work supporting the GP practice alongside a focus on PCN initiatives including novel, innovative at-scale work and opportunities. Additionally, they will provide enhanced services to the registered population.

Main duties of the job
  1. Assess and triage patients, including same day triage, and as appropriate provide definitive treatment (including prescribing medications following policy, NICE (national) and local clinical guidelines and local care pathways) or make necessary referrals to other members of the primary care team.
  2. Implement and evaluate treatment/interventions and care for patients presenting with an undifferentiated diagnosis.
  3. Clinically examine and assess patient needs including history taking, assess, diagnosis, plan, implement and evaluate interventions/treatments for patients with complex needs and onward refer where appropriate.
  4. Deliver high-quality healthcare to care home residents who often have complex medical needs, conducting thorough initial assessments upon admission to the care home.
  5. Use complex decision making to inform the diagnosis, investigation, complete management of episodes of care within a broad scope of practice.
  6. Prescribe and review medication for therapeutic effectiveness, appropriate to patient needs and in accordance with evidence-based practice and national and practice protocols, and within scope of practice.
  7. Assess, identify, and refer patients presenting with mental health needs in accordance with the NSF for Mental Health. Implement and participate in vaccination and immunisation programmes for both adults and children.
  8. Support the delivery of advanced care plans.

  5. Evaluate patients response to health care provision and the effectiveness of care.
  6. Supervise other HCPs including ARRS staff and help support their training.
Person Specification
Qualifications
Essential
  • Newly qualified GP, within 2 years and not having had a substantive post.
  • Registered with the General Medical Council (GMC) and license to practice.
  • Membership of appropriate Medical Organisation (MDU, MPS, etc.).
  • Inclusion in NHS England / NW London Performers List of Medical Practitioners.
  • Annual appraisal updates.
  • Experience of working in a primary care environment.
  • Experience of dealing with a range of clinical conditions.
Desirable
  • Understanding the unique health challenges faced by elderly patients, including frailty, polypharmacy, cognitive decline, and mobility issues.
  • Some exposure to palliative care and end-of-life decision-making is expected.
  • Experience of medicine management.
  • Experience working as part of a multidisciplinary team.
  • Experience in triage.
  • Knowledge of Quality and Outcomes Framework.
  • Knowledge of public health issues in the local area.
  • Awareness of issues within the wider health arena.
  • Knowledge of health-promotion strategies.
