Job description

IOCP is a nonprofit that provides services in food, case management, emergency financial assistance, employment services, and childcare resources to those residing in eight Western Hennepin County cities. We are looking for a Neighborhood Program Case Manager to play a key role in 2-3 housing communities providing individual services and community connection to residents.

The Neighborhood Program Case Manager conducts new client intakes, provides information and referrals, does 1:1 case management and creates meaningful community connections. What You Will Do

  1. Conduct client intakes with a focus on exceptional customer service and quality data collection through trauma-informed practices. Describe our services, collect information and assess presenting needs.
  2. Provide information and referrals, responding to the needs of clients with a trauma-informed approach and facilitate their connection to internal and external programs and resources. Assist residents to respond to crises.
  3. 1:1 case management including resolving crises, assessing needs, connecting to resources, developing and implementing plans to work toward increased stability and achievement of individual goals. Work with residents to assess strengths and barriers. Provide connections to external and internal resources, advocacy, counseling and problem-solving assistance in support of housing retention and stability goals.
  4. Community building such as community needs assessments, new resident welcoming, promotion of positive resident/community involvement and working with residents and community partners to coordinate community dinners and other community events.

What You Will Bring

  1. BSW preferred or Bachelor’s degree in a related field.
  2. 3-5 years direct service experience working with adults and families.
  3. Experience working in housing communities.
  4. Excellent interpersonal, verbal and written communication skills.
  5. Demonstrated experience working with populations of diverse age, income, race, national origin and disabilities.
  6. Commitment to equity and inclusion and ability to excel in a diverse environment.
  7. Knowledge of and ability to navigate community resources and systems.
  8. Ability to work effectively in teams.
  9. Self-motivated and able to work well with limited supervision.
  10. Excellent organization and time management skills.
  11. High level of computer proficiency.
  12. A valid driver’s license with a clean driving record and access to reliable transportation.
  13. Must be willing to travel between work sites.
