Position Type/Hours: Three Quarter Time/1200 hours
Service Location: South Carolina Alliance of YMCAs [candidates should live in one of the following counties: Oconee, Pickens, Darlington, Dorchester, Richland, Sumter]

PROGRAM PURPOSE:
The National Association of Chronic Disease Directors (NACDD) is a national public health non-profit agency focused on improving the health of the public by strengthening state and national leadership and expertise for chronic disease prevention and control. NACDD promotes health and reduces the burden of chronic disease through various programs and initiatives in collaboration with the CDC.
SPECIFIC SERVICE FUNCTIONS AND RESPONSIBILITIES:
- Support community health and wellness initiatives under the supervision of the Community Health Director.
- Examine statewide Community Health Impact via reviewing programmatic data and developing an improvement plan.
- Connect with the Community Health Director weekly to update on program status.
- Complete self-paced eLearning for each Y-USA Evidence Based Health Intervention.
- Travel to the local YMCA weekly to assist with various duties in the health and wellness department.
- Participate in all required Community Health team discussions.
- Complete necessary training to maintain active certifications.
- Assist onsite Program Coordinator with program outreach as needed.
DESCRIPTION OF ANTICIPATED SERVICE SCHEDULE:
- Hybrid service hours; 50% teleservice, 50% in-person.
- Service hours are to be completed Monday-Friday between 8:00am and 5:00pm, with potential evening hours.
- A maximum of 20% of service hours may be training or education activities.
- A maximum of 10% of service hours may be fundraising activities.
DESIRED SKILLS & COMPETENCIES:
- Knowledge and interest in public health/community integrated health.
- Ability to remain on feet for extended periods.
- Means and ability to travel to YMCA branches.
- Strong problem-solving, verbal, and written communication skills.
- Ability to work a flexible schedule, including nights and weekends.
- YMCA Group Fitness/Personal Trainer Certification preferred.
- HIPAA certification required.

REQUIRED COMMITMENT:
- Complete 1200 service hours within one (1) year.
- Service year begins 02/03/2025 and ends 02/02/2026.
ORIENTATION AND TRAINING:
- Pre-service orientation.
- PHA-specific training.
- Program-specific training.
EVALUATION AND REPORTING:
- Complete quarterly assessments and monthly reflection forms.
- Participate in quarterly performance evaluations.
- Submit service timesheets biweekly.
PUBLIC HEALTH AMERICORPS ELIGIBILITY AND REQUIREMENTS:
- Be at least 18 years of age.
- Have a high school diploma or equivalent.
- Be a citizen, national, or lawful permanent resident alien.
- Live within commutable distance of the Host Site.
OTHER PROGRAM ELIGIBILITY AND REQUIREMENTS:
- Willingness to comply with Host Site vaccination requirements.
- Preferred: Upper-level undergraduate or graduate student pursuing a public health degree.
