Job description

Under the clinical supervision of the Director of Social Work, utilizes clinical social work techniques and theory to provide consultation, assessment, treatment interventions and advocacy for a diverse population of patients and their families. Utilizes a biopsychosocial approach to healthcare, while collaborating with interdisciplinary teams; to provide case management and crisis intervention for a wide range of complex social factors and determinants of health within the department of Psychiatry.

POSITION SUMMARY:

This social worker will be under the supervision of the Director of Social Work, will function with a great deal of autonomy and with specialized expertise in the area of psychiatric services.

They will also work as part of the interdisciplinary team caring for patients in the department of Psychiatry.

II. Job Relationships: The MSW Social Worker reports to and is accountable to the Director of Social Work. The MSW Social Worker collaborates with all staff on the units in the medical center whenever needed for patient care total quality management, committees, focus groups, etc. The MSW Social Worker serves in a consultative role to staff on the units in the medical center whenever needed for patient care.

III. Authority: Provide social work services on unit(s), assisting patients and families to cope with plans and decisions that need to be made before, during and after hospitalization. Supervise and train graduate level social work students and other students when indicated and approved by the Director of Social Work and when the MSW has met the requirements for Supervision of the respective Graduate School of Social Work.

IV. A. Responsibilities/Essential Functions:

1. Provides superior customer service to internal and external clients, customers, and patients as referenced in the Service Excellence Standards.
2. Provide individual and/or group counseling/therapy to patients and/or their families.
3. Support patient advocacy.
4. Provide diagnostic evaluations and psycho-social assessments, treatment plans, discharge plans.
5. Provide referrals and/or follow-up services.
6. Collaborate with clinical interdisciplinary teams.
7. Facilitate continuing care plans.
8. Provide crisis prevention.
9. Supervise other social workers and graduate students.
10. Provide counseling for patients, family and friends whenever indicated at time of diagnosis, regarding adjustment to illness, the emotional impact of diagnosis, and changes in lifestyle.
11. Offer continuity by maintaining a supportive relationship to patient and family throughout the course of illness.
12. Counsel and support patients and families in bereavement when indicated.
13. Provide case management for patients, including resource findings, concrete services as indicated, and referrals to community agencies and assistance in obtaining financial aid.
14. Provide documentation for social work activity in patient's record on a regular and timely basis.
15. Consult with members of the medical center team regarding the psycho-social needs of patients and participate in staff in-service.
16. Lead or co-lead a support group for patients and/or family members when appropriate.
17. Participate in community organizations relevant to the population.
18. Make a psycho-social assessment of high-risk clinic patients as time allows, necessitating prioritizing problems.
19. See patients with histories of mental illness or retardation, child abuse or neglect, domestic violence, and serious substance abuse, following them through their pregnancies.
20. See patients at times of crisis and assist with needs for concrete services. Refer patients wishing to place their baby for adoption to the appropriate agency.

B. Responsibilities/Non-Essential Functions: The Social Worker will assess the discharge needs of all patients admitted to their respective department assignment.

V. Reporting Requirements: The social worker will report to the Department Director and works closely with the Patient Care Director of the assigned unit.

VI. Accountability: Shall be accountable for maintaining the confidentiality and security of all patient-related, medical-staff related, and patient-related data and information and for abiding by all relevant hospital policies and procedures.

JOB REQUIREMENTS

EDUCATION: Requires Master’s Degree (MSW) from an accredited School of Social Work to be obtained prior to employment.

CERTIFICATES, LICENSES, REGISTRATIONS REQUIRED: Must sit for the Massachusetts Licensed Clinical Social Worker (LCSW) Certification exam within the first 90 days from date of hire. Must obtain Massachusetts Licensed Clinical Social Worker (LCSW) Certification within 6 months of hire.

EXPERIENCE: Requires level of knowledge of Social Work process and community resources and agencies in relation to health, illness, and disability, generally acquired through one to two years of on-the-job training. Pre-MSW experience will be considered.

KNOWLEDGE AND SKILLS:

• Knowledge of community resources and agencies that serve adults, children, and families.
• Advanced interpersonal skills necessary to interact effectively with various healthcare professionals, to develop and maintain effective therapeutic relationships with patients and families.
• Must be comfortable in a fast-paced and dynamic work environment (sense of humor is valued).
• Ability to work independently and in an interdisciplinary team.
• Cultural sensitivity and comfort with a wide range of racial and ethnic populations. Bicultural/Bilingual competency a plus.
• Computer literacy with ability to use standard word processing, spreadsheet programs, and e-mail (i.e., MS Word, Excel, Outlook) and web browsers.
