Job Summary

An exciting opportunity has arisen for a band 6 mental health nurse to join the Hull Integrated Care Team for Older people, supporting adults over the age of 65, experiencing functional or organic mental illness, including dementia.

The post holder will support a multi-disciplinary team consisting of consultant psychiatrists, social workers, occupational therapists, nurse associates and support workers to provide a range of therapeutic interventions and evidence based approaches to meet the needs of the clients, promoting their independence and wellbeing.

The post holder will have experience of working with adults experiencing complex mental health needs and be able to provide clinical expertise and knowledge to the wider team.

Main Duties of the Job

To work within the multi-disciplinary team to meet the needs of clients and their carers under the care of the Community Mental Health Team for people over the age of 65 experiencing functional and organic mental health illness.

To support the Team Manager, Clinical Lead and colleagues in providing clinical and professional leadership.

To support the Team Leader, Clinical Lead and other colleagues with the implementation of policy, protocol, service/practice development and change within own area of responsibility.

To complete initial assessments and risk assessments to provide evidence-based, person-centred care planning to clients, considering their carers also.

To share own professional knowledge in team meetings to support client formulation and care planning.

The applicant will be expected to have specialist nursing knowledge of working with adults with complex mental health issues. Experience in assessing and managing clinical risk.

The post holder will manage a caseload of people with complex mental health presentations and will need to be able to prioritise their time effectively and maintain a degree of flexibility around their time management.

You will lead and participate in the development of staff through appraisal and clinical supervision.

Qualifications and Knowledge

Essential

  • Specialist knowledge across a range of interventions underpinned by theory
  • Professional knowledge acquired through degree and supplemented by specialist training and experience
  • Full understanding of relevant policy, legislation, drivers and their application to clinical and service area, e.g. Mental Capacity Act, Social Inclusion
  • Part III or 13 of NMC live register
  • Post registration/graduate diploma or equivalent experience in related work area, to demonstrate advanced level of practice
  • Demonstrable experience of mentoring pre-registration students
  • Prepared to undertake or hold a recognised non-medical prescribing qualification, if required

  • Evidence of project work that has impacted on practice and demonstrates a higher level of communication/planning/change that impacts within own area of work/responsibility
