Job Title: Mental Health Practitioner (SW/OT/RMN) – CNWL – Various Adult Mental Health Teams – Community Mental Health, Forensic and Psychiatric Liaison
Location: West London
Rate: Up to £31 per hour
Liquid Personnel is currently recruiting experienced and Qualified Mental Health Practitioners to join Mental Health Teams based in West London NHS Trust.
Responsibilities:
The primary duties of the roles (please note these will vary depending on the team) include providing high quality assessment and intervention to service users experiencing a wide range of mental health problems, promoting recovery and independence to identify appropriate pathways and treatment plans with service users. Responsibilities also include care planning, care coordination, managing a caseload in accordance with the Care Programme Approach (CPA), psychosocial intervention, risk assessments and supporting with Adult Safeguarding, working as part of MDT, actively participating in and contributing to supervision and team meetings, and maintaining a personal awareness of legislation, policies and procedures, particularly those relating to the Care Act, The Mental Health Act, the Mental Capacity Act, safeguarding and self-directed support in order to apply a strong evidence base to ensure consistency and a high quality of service delivery.
