Within this role, you will be involved in working alongside the MDT, working within the confines of a robust co-produced care plan that meets the individual service users' needs while managing any risk factors for the individual or others. In addition, this role will focus on proactively supporting reintegration back into the community, linking with third-party services such as housing and drug and alcohol services, and attending all necessary reviews and professional meetings as scheduled.
