This interesting role was created specifically to support our clinical teams, focusing on supporting our care for patients with Long Term Conditions like diabetes, asthma and COPD. You will also be responsible for ensuring that the medical records of our patients are kept accurate and clinically safe. Our clinicians will be able to practice safely, knowing that the data they are working with is of the highest quality and accuracy.
You will be part of a team responsible for identifying those that do not attend their long-term condition reviews and encouraging them to access a crucial part of their proactive care. In addition to this, the appointed person will support the safe prescribing of high-risk drugs by ensuring that patients on high-risk medications receive their regular monitoring reviews thus ensuring the prescribing of their drugs remains safe and the supply of their medication uninterrupted.

An exciting opportunity has arisen for an accomplished and highly motivated administrator. We are looking for someone to join our 5 strong GP partnership in our well-established practice based in recently built premises.
You will be part of our forward-thinking team to share our vision, willingness and drive to adapt to changes within the NHS.
We are a thriving practice with 14,000+ patients. You will be supported by a fantastic team including an experienced practice management team, 5 partners, 5 salaried and 1 retainer GPs, a strong and skilled nursing team, pharmacy team and mental health team and experienced frontline administrative staff. The practice is also supported by an embedded PCN team of pharmacists.
The appointed person will work within multi-disciplinary teams including the reception and admin teams and our pharmacy technician. The team is also directly supported by a GP, Practice Nurse and a Manager. However, it will suit someone who relishes responsibility and independent working.
If you have a desire to make a real difference to those living with Chronic Disease, want to work as part of a dynamic multi-disciplinary team and have an aptitude to learn we would love to hear from you.
Experience of working with patients and with SystmOne (clinical system) would be an advantage but is not essential. If you have experience working with the public and operating information systems, this experience is valuable and we would still encourage you to apply.
