Job summary

This newly created specialist nurse role will be instrumental in the Targeted Lung Health Check Project roll out, both across the trust and primary care.

As the link worker between all stakeholders, the post holder will co-ordinate screening MDT meetings, ensuring the flow of patient care is effective between all providers.

This role is suited to an individual who can deliver specialist care, knowledge, advice, information, and education to patients, carers, healthcare professionals, and relevant others; ensuring individual clinical, psychosocial, and emotional needs are met.

Highly skilled in sensitive and empathic communications, daily interactions for the post holder will deal with difficult conversations.

Main duties of the job
  1. To lead the development of the Targeted Lung Health Check Project across the trust and the primary care setting to provide high-quality, cost-effective service using research, audit, and evidence-based practice.
  2. To provide specialist care, knowledge, advice, information, and education to patients, carers, healthcare professionals, and relevant others ensuring individual clinical, psychosocial, and emotional needs are met.
  3. To participate in service delivery across the trust/primary care ensuring that it meets national recommendations and directives for patients with cancer.
  4. Manage the day-to-day delivery of the TLHC service and to be a link worker between all stakeholders and provide leadership and management for the Targeted Lung Health Check (TLHC) service. Co-ordinate screening MDT meetings keeping the flow of patient care effective between all providers.
  5. The post holder will work with the Lead Clinicians, in particular the Responsible Radiologist and Responsible Clinician for the TLHC Service in developing and implementing the local TLHC project in line with the National TLHC Protocol and Quality Standards.
