Job Title: Social Worker (Band 6)

Location: Central London

Duration: 3 months

Schedule: Monday to Friday, 9 AM - 5 PM

Start Date: ASAP

Rate of Pay: £27 per hour (umbrella, paid via direct engagement in a PAYE equivalent rate)

What You'll Be Doing

Join our team as a Social Worker and play a key role in supporting individuals in the Preparing for Adulthood and Transitions team. In this role, you will:

1. Perform independent clinical triage and facilitate social discharges from the acute setting.
2. Liaise directly with local authorities, ICBs, and internal stakeholders.
3. Support and assess community triage referrals from acute hospitals.
4. Provide guidance and mentorship to junior staff as needed.
5. Work in an office-based environment (no home visits required) at Barley Lane.

Requirements & Qualifications

To be successful in this role, you will need:

1. A current and valid registration with Social Work England (SWE).
2. Proven experience as a Social Worker, preferably in an IDH/D2A model.
3. Strong communication and interpersonal skills.
4. Ability to work independently and as part of a multidisciplinary team.
5. A commitment to delivering high-quality patient care.
6. A valid full UK driver's license (preferred but not essential).

Working Hours & Contract Duration

This role offers a flexible working position requiring availability Monday to Friday, 9 AM - 5 PM. The contract is for 3 months with a strong possibility of extension based on performance and service needs.
