Job Title: Prescribing Pharmacist

Location: London

Duration: Ongoing contract

Schedule: Monday to Friday, 9:00 - 17:30 (37.5 hours per week)

Start Date: April/May

End Date: Ongoing

Rate of Pay: £33 - £40 per hour

What You'll Be Doing:

Join our client's team as a Prescribing Pharmacist and play a key role in improving the lives of patients in a GP practice setting. In this role, you will:

1. Perform comprehensive patient assessments
2. Create and implement personalised care plans
3. Deliver evidence-based treatments to optimise patient outcomes
4. Collaborate with multidisciplinary teams to provide holistic care
5. Maintain accurate patient records
6. Prescribe medications independently in accordance with clinical guidelines
7. Conduct medication reviews and optimise patient treatment plans
8. Provide expert advice on prescribing and medicines management within primary care
9. Support patients with long-term conditions through appropriate medication management

Requirements & Qualifications:

To be successful in this role, you will need:

1. A current and valid GPhC registration
2. Experience working in primary care / GP practices
3. Independent Prescriber qualification (essential)
4. Strong communication and interpersonal skills
5. The ability to work independently and within a multidisciplinary team
6. A commitment to delivering high-quality patient care

Working Hours & Contract Duration:

This role is a full-time Monday to Friday position (9:00 - 17:30, 37.5 hours per week) and requires availability on-site. The contract is ongoing, with a strong possibility of extension based on performance and service needs.
