Remuneration: £450 per day + £15 per UDA for any over/under achievement

Description: Full or Part-time LOCUM DENTIST required for 2-5 days per week (FLEXIBLE) in a well-established INDEPENDENTLY OWNED practice near the affluent town of Owlthorpe, Sheffield.

• £450 per day + £15 per UDA for any over/under achievement

• Full NHS Performer Number if applicable
• Local Area Team confirmation
• GDC certificate
• UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years

• Clinical references covering places of employment over past 2 years (minimum 2 referees)
