Remuneration: £450 per day for 30 UDAs per day

Description: Part or full-time Locum dentist required for 2-5 days in a busy, INDEPENDENTLY OWNED mixed, well-established practice in Leicestershire.

1. Mon to Fri & Sat - 8 hour shifts between 8am to 8pm
2. £450 per day for 30 UDAs per day
3. Fully computerised, Pearl Software, Digital X-rays, Rotary Endo
4. Existing, well maintained list
5. Full support team including a Hygienist
6. FD training practice
7. Parking on site
8. Available now, start date negotiable

Compliance requirements:

• Full NHS Performer Number if applicable
• Local Area Team confirmation
• GDC certificate
• UK Blood test report showing Hep B Immunisation and Titre levels, Hep C, HIV and TB results dated within last 5 years
• Indemnity certificate
• Passport
• Enhanced DBS (within the last 12-36 months)
• Clinical references covering places of employment over past 2 years (minimum 2 referees)
• Degree certificate
• CPR/AED/Medical Emergencies certificate dated within the last 12 months
• IRMER
• PIN number
