Description:

Part time Locum Dentist is required for 2 days per week in an INDEPENDENTLY OWNED mixed practice near Birmingham, West Midlands.
• Tuesdays & Thursdays
• Hours 10am – 6pm
• £450 per day 30 UDAs
• Private & Labs 50% split
• Must be able to provide Root Canal Therapy
• Established list
• Therapist on site
• 2 surgery practice
• Digital X-rays, OPG & Bellmont Chair
• May be locum to permanent option
• Experienced nurse support
• Free Parking on site
• Available immediately, start date negotiable.

Compliance requirements:
- Full NHS Performer Number
- Local Area Team confirmation
- GDC certificate
- UK Blood test report showing Hep B immunisation and titre levels, Hep C, HIV and TB results dated within last 5 years
- Indemnity certificate
- Passport
- Enhanced DBS (within the last 12-36 months)
- Clinical references covering places of employment over past 2 years (minimum 2 referees)
- Degree certificate
- CPR/AED/Medical Emergencies certificate dated within the last 12 months
- IRMER
- PIN number.
