What you'll do

The Licensed Practical Nurse (LPN) Ambulatory Care provides direct and indirect patient care under the direction of the Registered Professional Nurse, Nurse Practitioner, Physician’s Assistant, or Physician. The LPN functions as part of the interdisciplinary health care team. May be required to make home visits.

  • Provides direct and indirect nursing care individualized to the patient’s needs.
  • Collects and records relevant patient health data accurately.
  • Evaluates health data to identify patient problems.
  • Implements and documents specified interventions.
  • Identifies deviations from expected patient outcomes.
  • Adheres to approved, research-based practice standards.
  • Performs and documents Point of Care (POC) testing and quality control in accordance with established laboratory policies and procedures.
  • Maintains and documents testing reagents/supplies and equipment according to established laboratory procedures.
  • Advocates for patients to ensure that care preserves and protects the patient’s autonomy, dignity and rights.
  • Delivers care in a non-judgmental and non-discriminatory manner that is sensitive to patient diversity.
  • Assures that patients have informed consent signed for indicated procedures.
  • Maintains current licensure, certification, and competencies.
  • Provides proof of current BLS validation to supervisor.
  • Completes required unit credentialing according to departmental policy.
  • Provides to supervisor evidence of completion all required annual training and competency validation.
  • Practices within the New York State-defined LPN scope of practice 100% of the time.
  • Acquires and maintains current knowledge and skills in his/her area of practice.
  • Performs other duties as requested and observed by supervisor or manager.

What we need from you

Education:

  • Graduate of a Practical Nursing program, required.

Experience:

  • Previous healthcare experience, preferred.

License/Certifications:

  • Current NYS LPN License and registration as a Practical Nurse or a valid interim permit, required.
  • BLS certification must be obtained within 6 months of hire, required.

Physical Requirements:

  • Heavy Work - Exerting up to 100 lbs. of force occasionally, and/or up to 50 lbs of force frequently, and/or up to 20 lbs, of force constantly to move objects.
  • Occasional climbing, crouching, grasping, kneeling, lifting, pulling, pushing, reaching, stooping, visual acuity (color, depth, perception), fit testing.
  • Frequent balancing, typing/clerical/dexterity, hearing, repetitive motion, seeing/monitor/computer use, standing, walking.

Hazards:

  • Occasional burns, chemical, electrical, mechanical, radiation/radiant energy.
  • Frequent bodily fluids/bloodborne exposure.

Working Conditions:

  • Occasional noise, dust, fumes, gases, mists, odors.

Travel:

  • Occasional travel - the job may require travel from time-to-time, but not on a regular basis.

Salary is based on a variety of factors, including, but not limited to, qualifications, experience, education, licenses, specialty, training, and fair market evaluation based on industry standards. Pay Range Minimum:

USD $24.74/Hr.
Pay Range Maximum:

USD $37.10/Hr.
