STAT Home Health in Monroe, LA is seeking PRN Licensed Practical Nurses (LPN)!


We invite you to become a part of one of the fastest-growing Home Health Care and Hospice agencies that spans across six states and employs over 2,400 healthcare professionals. Our vast network of healthcare providers allows us to provide optimal patient care and seamless transition across service lines.


Qualifications:
  1. Current Louisiana Licensed Practical Nurse (LPN) license required
  2. Minimum 1 year clinical experience as an LPN
  3. Current CPR Certification
  4. Compassionate and great time management skills
  5. Reliable transportation to conduct patient visits at their place of residence or facility

Job Duties:
  1. Provide nursing and supportive care in accordance with the Interdisciplinary Plan of Care, under the direction of the RN
  2. Communicate with the attending physicians, other home health team members, and other agencies as needed to coordinate optimal care and use of community resources for the patient/family.
  3. Document all services provided.
  4. Demonstrate knowledge of Medicare/Medicaid regulations for home health benefits.
  5. Report changes in patient condition and needs to RN
