As a Community Nurse Case Manager, you will:

• Conduct initial assessments in the community
• Train and assess workers' competencies and allocate support workers to patients
• Collaborate with the wider multidisciplinary team (MDT)
• Attend meetings and make referrals
• Manage a team of non-clinical case managers

• Full-time hours: 37.5 hours per week, Monday to Friday

The ideal Community Nurse Case Manager will:

• Be an RGN or RSCN with advanced clinical skills in complex care, including ventilators and tracheostomy management
• Have experience delivering care to both adult and paediatric service users
• Be a car driver, flexible to travel around the Birmingham/West Midlands area
